To complete the registration, you will need to visit the JCC for an introduction meeting, tour and finalize payments.

WAIVER, RELEASE, AND INDEMNIFICATION AGREEMENT

I, the undersigned, hereby make application for membership at The Mandel JCC of The Palm Beaches. I understand that fees are payable monthly, I may cancel my participation in the program at any time by providing a 30-day written notification and signed cancellation form to the membership department. Therefore, at the time of cancellation, a one final monthly payment will be charged. I understand that fees are not refundable or transferable.

I agree to be personally responsible for all fees and costs for any elected programs as well as the membership. In the event of non-payment, I will be personally responsible to pay all court costs and reasonable attorneys’ fees incurred in the effectuating collection.

Mandel JCC PROGRAM POLICY & LIABILITY WAIVER: Participation in any Mandel JCC activities and use of any recreational facilities involves a risk of accidental injury despite all safety precautions. Having been informed of the activities conducted at the Mandel JCC, I, as an individual or as a parent/ guardian of the participant or other user of such facility, assume all risks and hazards incidental to the activities and release from responsibility and agree to indemnify and hold harmless the Mandel JCC, its officers, directors, independent contractors, volunteers and all employees for any illness or injury to me or my children or family members or other participant or user of such facility occurring during his/her participation in any activities or use of any recreational facilities at/or conducted by the Mandel JCC.
